A CASE OF RELAPSING POLYCHONDRITIS PRESENTED AS BILATERAL SEQUENTIAL PERILYMPH FISTULA

A 21-year-old man visited the outpatient clinic with sudden aggravated vertigo which occurred several weeks ago, and also fluctuating ear fullness and tinnitus in his left ear. He showed spontaneous right-beating nystagmus and his vestibular function test implied typical left vestibular loss. His symptoms improved after absolute bed rest and an exploratory tympanotomy was performed under clinical suspicion. His symptoms improved further immediately after surgery. On the 16th day postoperative, his left auricle showed redness and swelling, oral antibiotics was prescribed in suspicion of perichondritis. On the 20th day postoperative, he visited with sudden vertigo, sudden hearing loss and auricular swelling of the right ear. On the TBCT, fluid density occupying the round window was observed in the right ear, and exploratory tympanotomy was performed. A piece of auricular cartilage was taken for biopsy and systemic steroids were administered with consultation to a rheumatologist. After the second surgery, dizziness, hearing loss, and bilateral auricular swelling were improved. After training for vestibular rehabilitation exercise, he was discharged from the hospital on the 9th day of surgery. We authors report an extremely rare case of relapsing polychondritis presented by inner ear symptoms associated with bilateral labyrinthine fistula.
